Whether you are a person living with a serious illness, a caregiver of a loved one, or the healthiest you have ever been, having an end-of-life plan in the event you are unable to speak for yourself is important.

In our Virtual Advance Care Planning Information Session, you will learn what you can do to provide yourself and your family with peace of mind knowing that your wishes will be honored no matter what happens. We will walk through:

• Identifying your health care priorities
• Discussing your preferences with loved ones
