In honor of National Healthcare Decision Day (April 16), we’re hosting two separate Advance Care Planning Information Sessions.
Whether you are planning for yourself or preparing to help a loved one make informed decisions, you’ll learn the ins and outs advance healthcare planning.
We will have printed Advance Health Care Directives (AHCD) available for you to take home, and will be happy to answer any questions.
Light refreshments will be served.
