Request Access to Your Family Member's MyChart Account

There might be a time where you need continual access to a family member’s medical record. If such a situation arises, you can ask your loved one to grant you proxy access to their MyChart account. If you’re a proxy, you will have access to your loved ones health records via your MyChart account. Additionally, notifications for your loved one, will show up in your feed.

You can request proxy access to manage a family member's healthcare from your computer or mobile device through MyChart. Once granted proxy access, you will be able to:

  • Message your family member's care team
  • See their lab and test results
  • Request prescription refills for your family member
  • Request appointments for your family member
  • Review their allergies, medications and immunizations

Follow These Steps to Request Proxy Access:

  1. Visit the MyChart page on our website. If you already have an account, enter your credentials to log in. If you do not have a MyChart account, click on the "Sign up for an account" button and fill in the requested information requested to create an account.
  2. Request access to your family member’s account: Click on the Menu icon in the upper left hand corner. Navigate to Sharing > Proxy Center - Request Family Access. Select the appropriate option (Parent/Legal Guardian Proxy - CHILD (0-11 years) or (Parent/Legal Guardian Proxy - ADOLESCENT (12-17 years). Fill in the requested information and send the request.

*Proxy access provides limited access for parents of teenagers (children ages 12-17). Teens may request their own MyChart access for full access to their account. Learn more about proxy access in the FAQ section on the MyChart web page.
