Medical Records - Medical Center
Please note: We are unable to accommodate walk-in patients and families requesting copies of their medical records in-person. Please see below to find out how to request different types of records.
How to request a copy of your medical records via MyChart:
The easiest way to request a copy of your medical records is through the MyChart app.
- Once logged on to your account, click on the "Menu" icon in the upper left hand corner.
- Scroll down the list of options in the menu to "Sharing" and select "Sharing Hub."
- Within "Sharing Hub" you can select whom you would like to have receive your records.
- Once you have selected who should receive your records, click on the "Request a Copy" button. A few fields of information will be requested:
- Make sure the "Where you received care" field is set to "Marin Health Medical Center"
- Select the date-range your request should cover

In the "message" box, indicate which fax number or physical mailing
address you would like to have your medical records sent to.
- If you do not indicate where the records should be sent, a copy of your records will be uploaded to your MyChart account.
- Click on the "Submit" button to send your request to MarinHealth's Medical Records Team.
How to request a copy of your medical records by mail:
Print and complete the Authorization for Disclosure of Health Inform form:

A patient over the age of 18 or their legal representative, may obtain a copy of their medical records by mail. The release form must be completed, dated and signed and a valid color copy of your photo ID attached. We ask that you specify what components of your medical records you wish to obtain. Often, the discharge summary, operative report and history and physical contain relevant information to suit your needs. In certain cases, a patient's physician, psychologist or social worker may also be required to approve a request made using a release form. Requests must be specifically signed/initialed if requesting/authorizing the following information: Psychiatric Care, Substance Abuse and/or HIV diagnosis or treatment.

Acceptable forms of original photo ID include:
- Driver’s license
- Passport
- Immigration Documentation
Release of information charges:
- No charge for up to 30 page count
- $0.25 per page 31 pages and over
If you have any questions regarding release of health information, please call 1-415-925-7270. Your request will be processed within 15 days. We will mail a copy of the medical records to the address you provide.
Imaging Results
If you would like a copy of your imaging results on a CD, please call our imaging services team: 1-415-925-7080. If your are calling after-hours, please leave a voicemail. We will arrange a time for you to pick-up your imaging results CD at our Radiology Office (lobby level) within MarinHealth Medical Center. It should take 10-15 minutes for us to create the CD for you once you arrive. Alternatively, if you need your results sent to a specific doctor or doctor's office, please be prepared to explain in detail which office or provider the results should be shared with. If you need your imaging records expedited, please let us know when calling.
Patient Rights
If you believe your health information was used or shared in a way that is not allowed under the HIPAA privacy law, or if you weren’t able to exercise your rights, you can file a complaint with your provider or health insurer. You can also file a complaint with the U.S. government.
A separate law provides additional privacy protections to patients of alcohol and drug treatment programs.
