Outpatient palliative care services offered through MarinHealth Supportive Care Center | A UCSF Health Clinic in Greenbrae help people with serious illnesses stay at home rather than having to go to the hospital. Palliative care reduces physical discomfort, improves quality of life, and gives you a say in your care. Palliative care makes living with a serious illness easier— both for patients and those who care for them.
We provide symptom management (pain, nausea, vomiting, fatigue, etc.), supportive counseling, spiritual care, connections to local resources, caregiver support, advance care planning, and much more. Care is based on a patient’s needs—not their prognosis.
You Can Have a Say in Your Care
Treatments only work if they work for you. Our team will work to ensure that the patient’s treatment plan aligns with their goals and values. With palliative care, patients and their caregivers are in the driver’s seat. You’ll be able to discuss what matters most to you and share in decision making to get the care that’s right for you.
An Extra Layer of Support
The specially-trained outpatient palliative care team at the MarinHealth Supportive Care Center includes a physician, nurses, a social worker, and a spiritual counselor, all of whom are here to assist with your concerns, and to help improve your quality of life as you receive treatment for a serious illness.
Our team also offers crucial support to families and caregivers, including education and emotional support to help those caring for the patient better understand the patient’s condition and reduce caregiver stress and burnout. Our team is here to help families navigate the complexities of the healthcare system, and can assist with accessing local resources.
Continuity of Care
Should hospitalization be required, our outpatient team coordinates with the inpatient palliative care team for a seamless transition to hospital-based palliative care.
